What Is Dry Needling?

Dry needling is a treatment where your provider inserts a very thin, sterile needle (no medication, hence "dry") directly into a tight, tender knot in your muscle, known as a trigger point. The needle itself, along with specific techniques and sometimes a mild electrical current, is what does the work, not any injected substance.

How It Helps You

Think of a trigger point as a small area of muscle that's gotten "stuck" in a contracted, oxygen-starved state, creating a knot that causes pain, tightness, and sometimes pain that radiates elsewhere. This treatment works by:

  • Mechanically releasing the tight, contracted muscle fibers at the source of your pain

  • Triggering a brief, involuntary muscle twitch that helps reset abnormal muscle tension

  • Increasing local blood flow and oxygen delivery to a muscle area that's been starved of both

  • Calming pain signals sent to your brain and spinal cord, reducing how much pain you actually feel

  • Kickstarting a mild healing response that helps repair irritated or degenerated tissue

Different Techniques Your Provider May Use

  • Scanning: Your provider moves the needle in different directions and depths within the muscle to locate the exact trigger point before treating it, similar to searching for the "sore spot."

  • Pistoning: Once the target is found, the needle is moved in and out repeatedly, like a small pumping motion, to provoke a "twitch response" from the muscle. This is often used for stubborn or chronic knots to help reset tension quickly.

  • Static (sustained) needling: The needle is inserted and left in place for a period of time without movement, offering a gentler option with less soreness afterward.

Adding Electrical Stimulation

Sometimes your provider will attach small wires from an electrical stimulation unit to the needles once they're in place. This combination:

  • Enhances muscle relaxation and reduces spasm more than needling alone

  • Stimulates deeper muscle fibers than manual needling by itself

  • Helps reduce post-treatment soreness in some patients compared to needling without stimulation

  • May improve motor control and muscle activation, useful when a muscle isn't firing properly

What You Might Feel

You may feel a brief pinch or ache as the needle enters, followed by a deep, dull sensation or a quick muscle twitch or cramp when the trigger point is found. If electrical stimulation is added, you'll likely feel a light tingling or gentle pulsing at the needle site.

What to Expect Afterward

  • Mild soreness for a day or two afterward is common, similar to how you might feel after a hard workout

  • Many patients notice improved range of motion, less muscle tightness, or reduced pain within a few sessions

  • Effects can build with repeated treatments, especially when paired with stretching or exercise
